NTU ID Card 10-Year Free Replacement Policy:
Effective immediately. Faculty and staff members who have held an NTU ID card issued by the Personnel Department for over 10 years, and whose card has been lost or is no longer functional due to wear and tear, may apply for a one-time free replacement, if necessary.

Application Procedure:
- Access the ID Card Application System
Log in to MyNTU → select “ID Card Application System”
- Apply for reissuance based on your employment category
Please follow the system instructions and complete the form according to your employment category:
- Full-time faculty/staff or contract employees
- Joint appointment, adjunct, visiting, clinical, or project-based faculty and staff members
- Project employees
- Upload a photo
Please upload a 1-inch or 2-inch front-facing photo for your ID card. The photo must be clear, taken without hats or colored glasses, and with fully visible facial features.
- System eligibility check for free replacement
- If the current valid card has been activated for 10 years or more → Eligible for free replacement.
- If the current valid card has been activated for less than 10 years → The system will generate an “ID Card Reissuance Application Form” → Proceed to the Cashier Division for payment → Upload the payment receipt in the system.
- Collect the new ID Card
Once the application is processed, please wait for notification to collect your new ID card at the Personnel Department.
★ ID Card Loss Procedure upon Resignation ★
When processing resignation procedures, faculty and staff members must return their NTU ID card. If the ID card is lost, please follow the steps below:
- Full-time faculty/staff or contract employees Enter “Student / Faculty and Staff ID Card Loss Report & Refund” system → Select “Report ID Card Loss” → In the function menu, select “Card Loss” → Complete the form as instructed. The system will automatically determine your card’s activation status.
- If the current valid card has been held for 10 years or more
→ No payment required
→ An “ID Card Reissuance Application Form” will not be generated.
- If the card has been held for less than 10 years
→ Payment required
→ An “ID Card Reissuance Application Form” will be generated
→ Please pay the fee at the Cashier Division, then submit the receipt to the Retirement, Pension and Insurance Division of the Personnel Department to proceed with resignation procedures.
- Project employees
Please additionally fill out a Declaration Form and submit it to the Integrated Affairs Division of the Personnel Department.
- If the current valid card has been held for 10 years or more
→ No payment required
- If the card has been held for less than 10 years
→ Payment required
→ Pay at the Cashier Division, then submit the receipt to the Integrated Affairs Division of the Personnel Department to proceed with resignation procedures.

Please log in to the “Student / Faculty and Staff ID Card Loss Report & Refund” system → Under “List of Valid ID Cards”, you will find your ID card activation date.
